Records Section will only accept cash, money orders, cashier checks, or business checks made payable to MARICOPA COUNTY SHERIFF’S OFFICE. No personal checks, debit or credit cards are accepted for payment. Payment must be presented in person or via U.S. mail.

HOW TO OBTAIN COPIES OF REPORTS:
Location History, Incident History, and Crime Statistics Reports can only be delivered via email by
submitting a records release form.
You can pre-order Victim Reports by submitting a records release form via email.
For a Departmental Report, please contact 602-876-1078 to verify availability and cost of the report. If it is available for release, print a copy of the request form and mail the request and payment to the below address.
Traffic accident reports are $5.00
for the first ten (10) pages and $.50 per page thereafter. Please contact the Departmental Reports Section for report cost and availability prior to sending a request or arriving at the Maricopa County Sheriff's Office to obtain the report.

Traffic Accident Reports, Departmental Reports, and Victim Reports cannot be faxed (or emailed) except to law enforcement personnel or prosecutors office.
You may personally pick up Traffic Accident Reports, Departmental Reports, and Victim Reports at:

Please contact 602-876-1078 prior to picking up reports to ensure availability of the report.
Parking:
There are metered parking spaces on the street and a fee for parking garage in the vicinity.
You will need to enter through the East Court Security area. Once through security travel south past the elevators until you reach the main hallway. Turn right at the hallway and travel west until you come in front of Sheriff’s Office entrance. The entrance is directly across from the Change of Venue cafeteria entrance.
You can pre-order Victim Reports by submitting a records release form via email, or by US mail (Please include the DR# as it will help for quicker processing). Victim reports are free for the first copy to the victim him/herself or an immediate family member if the incident resulted in an incapacitating or fatal injury.
A victim is defined as a criminal matter that has happened against you (i.e. Assault, Burglary, Robbery, etc.). You cannot be a victim of an incident if you were cited and/or arrested for the offense.
If you are requesting a Departmental Report (DR), please contact 602-876-1078 for report cost.
$5.00 for the first ten (10) pages and $.50 per page thereafter. If the report has been received by the Departmental Reports Section, you will be advised of the cost. If it has not been received, you should contact the above number at a later date to determine the cost prior to submitting a request. Reports are generally received by the Departmental Reports Section within 7-14 business days (Monday – Friday 8:00 A.M. – 5:00 P.M. excluding holidays) after the incident has been reported.
Please note that the Maricopa County Sheriff’s Office only has reports that occurred within MCSO’s jurisdiction. We do not have reports for other agencies such as Phoenix PD, Mesa PD, etc. If your incident occurred in another jurisdiction other than MCSO’s, please contact that agency for a copy of the report.

All citizen generated Calls for Service (CFS) and Officer Initiated (OnView/OV) incidents that Sheriff’s Deputies respond to will have an Incident History Record. The Sheriff’s Office maintains Incident History for the preceding ten years plus the current year. Incident History Reports can only be provided for areas that are serviced by MCSO. You must contact your local law enforcement agency if you are requesting information that falls within incorporated municipalities not served by MCSO.
An Incident History Report is the actual Computer Aided Dispatch information that was typed by a call taker and dispatcher at the time of the incident.
It contains limited information obtained during the incident. Such as:
Dates and Times of occurrence
Nature of Call (Burglary, Stolen Vehicle, Fight, etc…)
Unit(s) and Sheriff’s Office personnel responding to incident
Information/Remarks/Comments obtained from Complainant and Deputies at the scene
Does not indicate if someone is guilty or innocent of a crime.
It will indicate if a Departmental Report (DR) was taken. All Incidents are not required to have an associated DR submitted.
The information contained in these documents may be unsubstantiated and incomplete. They are likely to contain information, which may be deemed sensitive or embarrassing and have emotional impacts on the principals.
Information may be edited/redacted before release to requestor in order to protect privacy.
Requestor must complete a Records Release Form. The request must include an existing Incident History number or address/location where the incident occurred, time frame when it occurred, and any additional information that will help to locate the incident.

Location History reports provide a brief summary of activity for a specific location(s) over a period of time. MCSO does not provide Location History Reports for neighborhoods or large geographic areas. See Crime Statistics. The Sheriff’s Office maintains Location History for the preceding ten years plus the current year. Location History Reports can only be provided to areas that are serviced by MCSO. You must contact your local law enforcement agency if you are requesting information for a location that falls within an incorporated municipality not served by MCSO. Requestor must complete a
Records Release Form. The request must include an address/location and time frame.
The report provides basic information that includes
Incident/Report (DR) number
Address/Location
Date Reported
Time Reported
Type/Nature of Incident
Disposition/Final Outcome of Incident (whether or not a report (DR) was taken)

Crime statistics can only be provided for areas that are serviced by MCSO. You must contact your local law enforcement agency if you are requesting information for a location that falls within an incorporated municipality not served by MCSO. The Sheriff's Office does not have the capability to provide crime statistics based on zip codes. However, a requestor can submit a specific location and we will provide crime information for that surrounding area based on current reporting area boundaries. Requestor must complete a
Records Release Form. The request must include an address/location and or north/south and east west street boundaries. Requestor must have Adobe Acrobat Reader installed to read the report.
